By purchasing a fitness/nutrition plan from Nuvu Fit I agree and consent to the following:

 

I am voluntarily participating in the training program I have purchased to include workout and nutrition.  I recognize that the program requires physical exertion that may be strenuous at times and may cause physical injury and I am fully aware of the risks and hazards involved. 

 

I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the above mentioned program. I represent and warrant that I have no medical condition that would prevent my participation in the program.

I understand that the nutrition is recommend and not required. I am held completely liable for the food choices I make.  I understand that I should observe any allergies and make modifications based on my own health needs. I hold SynergyNuvu,LLC and all affiliates blameless.

I agree that this program plan is only as great as I make it.  If I do not see results it is up to me to communicate that to my coaches. However, results are only guaranteed if I agree to follow the plan 100%.

I understand that there are no refunds and all plans are designed in weekly or monthly session blocks. If I sign up for reoccurring plans I understand that I will be billed automatically unless I cancel prior to the next training start date. If I do not cancel I will be billed for the next training cycle. I may upgrade to other levels within the first 7 days of my plan.

 

I agree to assume full responsibility for any risks, injuries or damage know or unknown which I might incur as a result of participating in the program. Such injuries may include, but are not limited to, heart attacks, muscle strains, muscle pulls, muscle tears, broken bones, shin splints, heat prostration, injuries to knees, injuries to back, injuries to foot, or any other illness or soreness, including death.

 

I knowingly, voluntarily and expressly waive any claim I may have against Nuvu Fit or any part/association/affiliates of SynergyNuvu, LLC for injury or damages that I may sustain as a result of participating in the program. 

 

I, my heirs or representatives forever release, waive, discharge and covenant not to sue any part/association/affiliates of SynergyNuvu,LLC for any injury or death caused by their negligence or other acts. 

 

I have read the above waiver and release of liability and fully understand it contents. By continuing with the program I voluntarily agree to the terms and conditions also stated.
